GPs, if you're interested in working as a locum in Greater Manchester, we need the following details and documentation from you to register as a locum with us:

• Your bank sort code and account number for direct payment (we can send a cheque if you prefer)
• Copy of JCPTGP certificate
• Copy of your CRB check
• Copy of CV with two referees listed
• Copy of GMC registration
• Copy of MDU/MPS insurance certificate
• Copy of letter of inclusion on supplementary or performers list
